Job Summary
This is not a traditional nursing role.
The Triage RN serves as a central hub between patients, providers, pharmacies, laboratories, and specialty testing facilities. Success in this role requires strong clinical judgment, exceptional organization, comfort with technology, and the ability to manage a high volume of patient communication while maintaining a 5-star patient experience.
While there is some direct patient interaction, the majority of the role is computer-based and focused on triage, clinical coordination, documentation, care plan implementation, and provider support.
This position is ideal for nurses who enjoy critical thinking, problem-solving, and managing complex clinical workflows. It is less focused on bedside nursing and more focused on being an operational and clinical quarterback for patient care.
What This Role Actually Looks Like
A typical day may include:
- Managing and responding to 100+ patient portal messages
- Reviewing symptoms and determining appropriate next steps
- Processing medication refill requests
- Coordinating specialty laboratory testing
- Navigating multiple specialty pharmacy and laboratory portals
- Entering medications, supplements, laboratory orders, and treatment plans into the EHR
- Following provider-established protocols and care plans
- Collaborating with providers regarding patient concerns and escalations
- Assisting with phentermine monitoring appointments
- Supporting pellet procedures and other clinical workflows
- Maintaining detailed documentation and follow-up tracking

Responsibilities
As a Triage RN, you will serve as a key link between patients, providers, pharmacies, laboratories, and specialty testing companies. You will play a critical role in ensuring timely communication, coordinated care, and an exceptional patient experience within our functional medicine model. Your responsibilities will include:
Clinical Triage & Patient Care Coordination
- Review, prioritize, and respond to a high volume of patient portal communications
- Use strong clinical judgment to assess patient concerns and determine appropriate next steps
- Escalate urgent or complex clinical matters to providers as appropriate
- Support patients throughout their care journey with timely follow-up and education
- Assist providers in implementing individualized functional medicine treatment plans
- Coordinate care recommendations, testing, and treatment plan execution
Medication, Laboratory & Testing Management
- Process prescription refill requests according to provider-approved protocols
- Support specialty pharmacy prescriptions and ongoing patient monitoring
- Submit laboratory orders and coordinate specialty diagnostic testing
- Navigate specialty laboratory and specialty pharmacy platforms
- Enter medications, supplements, laboratory orders, and treatment plan components into the EHR
- Complete referral forms, prior authorizations, and clinical paperwork as needed
Provider Support
- Communicate clinically relevant information and patient updates to providers
- Prepare charts and ensure providers have necessary patient information available
- Support clinical workflows, including phentermine monitoring visits and pellet procedures
- Assist providers with documentation and care plan implementation
Clinical Support & In-Office Responsibilities
- Assist with rooming patients as needed
- Obtain vital signs, InBody scans, and symptom assessments
- Perform limited phlebotomy and IV support as a backup resource
- Assist with regenerative and aesthetic procedures as needed
- Support clinic flow and maintain a 5-star patient experience
Team & Workflow Excellence
- Maintain accurate, detailed, and timely documentation in the EHR
- Collaborate effectively with providers, health coaches, PSRs, and administrative staff
- Contribute to process improvement and workflow optimization
- Participate in onboarding and training of new team members
- Take ownership of assigned responsibilities and follow through to completion

Qualifications
We are looking for candidates who meet the following qualifications:
Active Registered Nurse (RN) license in Wisconsin
Minimum of 3 years of nursing with triage experience
Strong clinical assessment, triage, and critical-thinking skills
Exceptional organizational and time-management abilities
Comfortable managing a high volume of tasks, patient communications, and competing priorities throughout the day
Strong computer skills and the ability to navigate multiple systems, portals, and software platforms efficiently
Excellent written and verbal communication skills
Ability to work independently while collaborating effectively with providers and team members
High attention to detail with a commitment to accurate documentation and follow-through
Comfortable learning new protocols, medications, laboratory testing, and functional medicine concepts
Experience with EHR systems and electronic patient communication platforms strongly preferred
Experience in functional medicine, integrative medicine, primary care, family practice, endocrinology, obesity medicine, or care coordination is a plus
Comfortable performing basic clinical support activities including rooming, vital signs, phlebotomy, and assisting with in-office procedures as needed
