Job Summary

Wisconsin Institute of Functional Medicine (WIFM) seeks an experienced, skilled, compassionate, and results-driven Health Coach to join our integrative healthcare team. This part-time role is pivotal to our mission, helping patients implement sustainable lifestyle changes, understand their treatment plans, and stay engaged throughout their journey to better health. The ideal candidate is warm, professional, highly motivated, and comfortable holding patients lovingly accountable. You’ll serve as both a cheerleader and a coach—supporting each individual with empathy while also encouraging real progress and follow-through.

Responsibilities

As a Health Coach at WIFM, you’ll play a key role in supporting our mission to provide personalized, root-cause healthcare. With your steady expertise and compassionate approach, you’ll help ensure a seamless, comfortable, and professional experience for every patient. Working alongside our collaborative clinical team, you’ll contribute directly to accurate diagnostics and exceptional care. Your responsibilities will include:

  • Provide personalized 1:1 coaching sessions to support patient lifestyle and behavior change goals

  • Guide patients through their care plans with clarity, empathy, and accountability

  • Identify challenges, celebrate progress, and motivate patients to stay on track between visits

  • Develop individualized coaching strategies aligned with clinical treatment plans

  • Collaborate with providers to ensure continuity of care and shared goals

  • Educate patients on nutrition, exercise, stress reduction, sleep, and functional medicine principles

  • Follow up with patients via phone, portal messages, or in-person to ensure engagement

  • Maintain accurate and timely documentation of all interactions in the EHR

  • Support resource development (e.g., patient-facing education, health blogs)

  • Develop and propose patient-friendly programs to enhance engagement and education, such as community events, workshops, or content for clinic newsletters

  • Participate in weekly team meetings and contribute ideas for patient success

  • Serve as a positive, dependable, and adaptable team member in a fast-paced clinic

  • Flexibility to work alternative hours as needed

  • Other duties as assigned

Qualifications

We are looking for candidates who meet the following qualifications:

  • Certification from an accredited Health Coaching program (NBHWC, FMCA, IIN, etc.)

  • 2+ years of experience in health coaching, patient care, or related field

  • Preferred: Bachelor’s degree in nutrition, psychology, exercise science, or health sciences

  • Background in functional medicine, gut health, and/or personal training is a major plus

  • Must be passionate about holistic health and root-cause medicine

  • Excellent interpersonal communication skills
